  • Please complete this form for each adult or youth attending the conference in your party. There is a separate form for children (up to grade 6) attending the conference.
  • After you complete this form, you will need to proceed to your "shopping cart," then to "checkout," and from there to PayPal.com, where you will enter your payment information (unless you are paying by check).
  • After you complete checkout, the system will display a confirmation statement and email you a receipt. Until you see this confirmation, you are not registered!
  • For youth registrations, we also need to receive a signed Parental Permission Form and a Youth Conduct Agreement by mail.  Download the forms now or download from the links in your confirmation email after you register.  Please print, sign, and mail them so they are received by the registration deadline of 4:00 PM on April 4, 2012.

Workshop Session A

Saturday morning at 11:00. Session descriptions
In case your first-choice workshop is unavailable, you will be assigned your second choice.

Saturday afternoon at 1:45. Session descriptions

If your first-choice workshop is full, you will be assigned your second choice.
Would you like to sing in the choir on Sunday morning? If so, please indicate which part you sing. The choir will rehearse during Workshop C on Saturday and after breakfast on Sunday.

Options

A limited number of beds are available in congregants' homes. Please indicate if you would like to be considered as a guest. A goodwill donation in an amount that fits your budget is expected from home hospitality guests.
If you can provide a ride to the conference for others, or if you need a ride, please briefly indicate how many passengers you can carry and any special considerations about travel time, etc. You do not need to specify your location or contact information here.
If you would like to share a hotel room with someone other than your own party, briefly describe your needs. Example: "Female night owl, have room already, no snorers please." Please note that many people wait until right before the deadline to register for the conference, and they will not be visible as needing roommates until they register. You are encouraged to go ahead and reserve a room to ensure that you will have a place to stay, and then cancel your reservation if you do not need it.
Please indicate any accessibility needs you may have, such as wheelchair access, allergies, hearing or visual impairment, so that we may do our best to accommodate you.
