This page provides quick links to all of the forms that Prairie Star District maintains on line. The idea is that you can download these forms and print them on your own printer, fill the forms in, and mail them into the District Office (or to another address that may be specified on the form).

- Prairie Star District is always looking for volunteers for District positions, both elected and appointed. Please help by suggesting a potential candidate. You may fill out this form for yourself or anybody else, with or without his/her permission. This information will be kept on file and the person contacted when an appropriate opening appears.
- Award Nomination Forms
- Forms for nominating people for various PSD awards. Nomination forms must be submitted by January to be considered for the next Annual Conference.

- Sponsored Event Application
- The District encourages congregations and UU organizations to develop and conduct conferences and workshops that help to promote UU values, provide opportunities for personal growth, and help to improve the quality of life in the congregations. Any workshop or conference which carries the District name and/or seeks District sponsorship must be approved by the District Board on an annual basis.
