The PSD Expense Form is used by PSD staff and volunteers to report personal expenses incurred while engaged in District activity. You can print as many of these as you need.

Note: To save a PDF file displayed in your web browser, you must use the Acrobat “Save” icon with the picture of the computer diskette, not the browser’s “Save” or “Save As” command in the “File” menu. You can also save the PDF file by right-clicking on the link to the expense form above and selecting “Save Target” or “Save Link” from the pop-up menu.
Instructions
- The form is designed so that you can fill in the blanks in Acrobat Reader, and print the filled-in form directly from Acrobat Reader. (You can also print blank forms and fill them in by hand.)
- Any information you enter on-screen can be printed, but will not be saved when you save (or email) the document. The on-line editing is only a printing convenience. So save the original form, and each time you need to fill it out either (1) print it and fill it out by hand, or (2) open it in Acrobat Reader, fill it out on-screen, and print it.
- If you want to fill the form in on-screen, be sure Acrobat Reader is not in Select Mode: the cursor should have the shape of a hand
(which will change automatically to a vertical line when you mouse over a field that can be filled in). There is a button in the Acrobat toolbar with the hand icon on it—clicking on this button takes Acrobat out of Select Mode.
- You can check on-screen the appropriate check-box for your District role (Board | Staff | Group).
- Any mileage you enter on-screen in the mileage field will be multiplied automatically by the District-approved mileage rate.
- Expense amounts you enter on-screen will automatically be totalled.
- If you want to start over (for example, to fill in a second form after printing the first), press the “Reset Form” button at the top of the form.
The completed forms should be mailed to the appropriate person (your Committee Chair, Program Area Coordinator, or other supervisory staff) for approval. After approval, the form should be forwarded to the District Office.
